  • Middle Name - the Govt Application form (OCI services) does not ask, so always blank.

The way the web based application works, it does not provide a field for entering Middle Name, but the final document that is generated shows my Middle Name as Blank. Even if I enter the Middle Name next to the First Name field (similar to US Passport), the Registration Form still puts the Middle Name field blank and enters that name next to the First Name (I tried both ways).

The VFS form that is filled out recognizes that US Passport is different, and specifically asks to enter a Middle Name, unlike the Govt. Form.

If I just write on the printed form it may not match what is in the system. It may still not match what is on the VFS system.

I'm not sure what to do here. What's the advice?

Govt appln form asks for only "Surname" and "Given Name".
Enter your last name for "Surname" and everything else under "Given Name" - EXACTLY as it was on your Indian passport / Surrender Certificate.

Don't write anything on the printed form. It is what it is. Thousands are applying using the same forms & process, and they know how to handle the variation.

Also, there is NO middle name field in the printed form (for OCI regn), so not sure where you are seeing the blank field.

VFS form does not matter - it is only for fee payment and tracking. Your actual OCI appln is the one used to process your service.
