Indian Passport Renewal Help

Dear Sir/Madam,

I currently reside in USA,MICHIGAN . My passport has been expired.

I have a few questions regarding my Indian Passport Renewal .

1) Indian Address

My Indian address is changed.I have Indian Drivers License with the new address,When i renew my passport will this proof suffice with notarization

2) Lost passport section

Do i need to fill the Lost passport section(Section 22) in the application form

3) US Address Proof

Will AAA insurance Letter or a letter from my Current Employers HR stating my current USA address serve as Address Proof

4) The total Amount I just wanted to confirm whehter my total amount is 80$ when i send it thru post Please let me know if there is any number to contact you regarding any doubts.
