Need OCI. Only have copy of passport

I am applying for OCI. I cannot find my Indian Passport. But I have a
copy duly stamped ''CANCELLED" Whether that would do it. Or I have to
provide affidavid, if so what is format of affidavid.

Do I have to submit USA passport alongwith Form A & B for OCI.

OCI sticker for old US passport

After we applied for the OCI card for my son(Minor), his US passport renewed. So now he has new US passport and also the old US passport.

I went to the Indian Consulate to get the OCI certificate and OCI Visa sticker on my son's old US passport so I can use Misc. OCI service to apply for his OCI visa transfer to new passport.

But I was denied to get the sticker or OCI certificate.

Without the OCI certificate and OCI visa sticker on his old US passport, I cannot apply using Misc OCI services for his OCI visa transfer to new passport as instructed on the website.

Any idea? Help?
