Hello,
I am currently residing in Raleigh, North Carolina on a F-1 . My passport expired on August 1, 2010. It had a validity of 5 years and hence I need to renew my passport. I have filled out the online application and I also got the appointment date and time. I am planning to send the form and other related document by mail.
Relating to the supporting documents, I have these following queries:
1. My home address in India has changed since my old passport was issued and hence I want the new passport to have my new address. Do I need to fill out another form or do I need to pay the amount of 10 $ for change of address? What about proof of residence ?
2. For proof of residence, do I need to submit address proof for my US address or Indian address?
3. The cheque for sending the fees and return service can be any bank's cheque?
