Indian Passport Renewal Help

Dear Sir/Madam,

I currently reside in USA,MICHIGAN . My passport has been expired.

I have a few questions regarding my Indian Passport Renewal .
1) Indian Address. My Indian address is changed.I have Indian Drivers License with the new address,When i renew my passport will this proof suffice with notarization

2) Lost passport section.Do i need to fill the Lost passport section(Section 22) in the application form?

3) US Address Proof - Will AAA insurance Letter or a letter from my Current Employers HR stating my current USA address serve as Address Proof

4) The total Amount -I just wanted to confirm whehter my total amount is 80$ when i send it thru post.

Dear Sir/Madam,

I would like to know one more information

Do i need to send the original passport when i send the documents by mail and if i need to send the original passport then do i need to send the photocopies of the first 3 pages and last 2 pages of the passport with notarization ?

Or is it fine if i send the notarized copy of first 3 pages and last 2 pages of the passport ?
