Incorrect Place of Birth on PIO Card

I had applied for my Son's PIO card at SFO Indian Embassy. I got the PIO card today. But my son's place of birth is incorrect on PIO card and is not matching with his DOB certificate.

Please advise what to do.

Does the place of birth in PIO match his passport? Its supposed to match the passport, and not the Birth Certificate. Still if its wrong, send the PIO to the consulate with a covering letter; they'll correct it for you..

Place of Birth on PIO card does not match with US Passport. I also sent an e-mail thru SFO consulate website and fortunately I got the reponse stating that I need to send the PIO card back along with a letter to them for correction. Now my questions are:
Apart from PIO card & covering letter, do i need send the following:
* self addresses USPS express mail
* Original Passport
* DOB certificate

Please advise. Thanks a lot in advance.

My sons PIO card has his last name incorrectly mentioned on the last page. His name is correctly mentioned on the front page. Does this error warrants sending the PIO card back to Consulate for correction?
