I have left surname blank in passport application form.I added surname in the main name column itself. I applied before 2 days. now how can I change or add surname to that? is there any possibility to do so?please reply me as fast as possible .

Make a new Application

Please make a new application correcting your mistake.

Fax and courier a covering letter, copy of the new application and a copy of the old application. Explain the details in the covering letter.

The Fax number will be listed on the consulate website.

Follow up with phone calls, emails and fax - to make sure the consulate understand your letter and correct the mistake.
