    Hello,
    my passport is expired and applying for renewal of passport. attached you find the latest affidavit for change in appearance and signature form. I have taken this form to the AAA office and Other Notary places in State Of Pennsylvania to notarize. but they refused to notarize it because there is no spot on the form for Notary and put their notary stamp. Public Signature.

    Please assist me with this​

    I just applied for my dad's passport and we had that page notarized but we are in CA. They had their stamp with same wording that is usually on notary documents and he then filled in the rest with his notary details. Ask them if they can attach another page with acknowledgement. Try UPS or some other local store. I had to explain what we were doing because he gave me the weird look and asked what was it...
    Ours had similar wording

      I have tried few places and no one in state of Pennsylvania is notarizing this documents because there is no spot for notary to put there stamp and sign. and if we do notarize we will loose our stamp and notary license and we can't just stamp anywhere there's got a place for us to put stamp.
      why Indian government did not add (Signature of Notary Public) and (Notarize Seal) if they wants notarize the documents.
      same thing happen when I tried to notarize green card and state issued picture id.

        For DL and Green card, we copied in color as stated and attached an affidavit stating " I, 'Name' certify that this is true and correct copy of my driver's license. License #. It's called a California jurat with affidavit statement here. Even ups has declined to notarize? If they all deny try going to indian agent that have small office maybe they'll notarize it?
