My wife is applying for 10 year renewal of her passport (Indian) and also wants to change her last name.
1. Do we need to pay $40 or $80 as both services are mentioned separately on NY consulate page, each costing $40?
2 Only proof of residence in her name is our joint checking account statements. Is that acceptable?
3. I should provide only a copy of my passport and not original passport along with marriage certificate. Is that correct?
