Convert PIO INTO OCI

Dear Customer service Manager,

I have some questions on behalf of my mother . Could you pleas answer
those questions . It may help us lot too.

1.My mother has PIO Card that was issued 5 years ago.
2.Is it valid document to travel to INDIA without additional requirements?
3. Does she need to surrender her old Indian passport?.
4. Can She convert into OCI ?
5. What should she do for this conversion ?
6 Does she need Indian Visa for traveling to INDIA now ?
7. Does she need to carry all old and new passports with her while
traveling to India ?
