Renunciation of Indian Citizenship with Passport - Two Questions

Hello, I have two questions about Renunciation Application.

I have recently submitted an application for Renunciation of Indian Citizenship with Passport at VFS New York. I got response from them to submit ' Name Change Affidavit ' document since my first name on Indian Passport appears differently from US Passport.

I have not changed my name but first name on Indian Passport has my full middle name included (like - 'ARPAN PRAKASHKUMAR) , while on US passport it is just first name ( like - ARPAN).

Questions :

1. Is there a name change affidavit template for this case ?
( I have found one at VFS website - https://services.vfsglobal.com/one-p...-affidavit.pdf. However I do no think it is applicable to my case since I havent actually changed my name but just appears differently on Indian Passport and US Passport )


2. I had not submitted 'Renunciation Certificate' with my application and in response from VFS it is not mentioned as missing either. Do I need to send it ?

Send one and same person affidavit notarized and you should have also sent renunciation certificate.
