OCI Renewal After 20yrs

Hello I am in starting the process to renew my OCI card with my new passport. I have filled out the application on OCI website and on VFS GLobal, I am in the process of collecting all documents required to mail with application. From my understanding they do not require USA Passport, just a copy with notarized form. Do I need to mail the original OCI card and the Visa on the old passport as well? For people who have done this process, can you kindly provide what all documents you sent along with the applications?

I did this process for my daughter last year and do not remember all the documents but the application process had fairly clear instructions on what to upload on the OCI site and mail in. Did you not get any instructions along the way?
