    Hello!

    I wanted to check if everything is correct and ask if anyone else has experience with changing your name on OCI after marriage.

    I changed my last name after marriage and wanted to get an updated OCI card. While I do see the checklist online, there are some things that aren't clear.

    Do I have to submit an affidavit of name change? I haven't had to for any other documents and only my marriage certificate worked. I have a copy of the marriage certificate with the original application. I used my current name on the application online so I'm not sure.

    Also, do I need to send my original OCI card? It states a copy of the first and last page is needed but I'm getting conflicting info if the original one is required.

    And does every document need to be signed at the bottom?

    Any other advice is helpful.

    I guess you have the passport in the new name first. So, apply in the new name. Your name in the passport and the OCI have to be the same. You mention (type it in) your old name in the reason for availing the service. I am not 100% sure if you need any other documents other than the marriage certificate. I hope someone else will answer that. You will be filling spouse's information, so you need to attach your spouse's OCI copy.

In case of change of name, previous name must invariably be mentioned in the OCI application, failing which the OCI application is liable to be rejected. (The document must be self-attested).

    The document checklist says you need the original OCI and copies of the first two pages and last two pages. This is not worked clearly. If you take a copy of the passport's first page, you also take it with the second page. Same with the last page.

    Self-attest all copies. Notarize the affidavit in lieu of the original form and any other document that has a notary mentioned.

      Yes, you should provide an affidavit of name change and the original OCI card for verification. All documents should be signed by you and your spouse.
