I am planning on applying for a PIO card for my daughter as her Indian Visitor's Visa expired along with her Passport. She is a US Born. This is what I would like to summarize. Please let me know if I am missing anything.
1. Copy of Birth Certificate ( notarized ?)
2. Copy of current US passport 1st 4 pages and last 3 pages. (notarized)
3. Copy of both parents Indian Passports (notarized).
4. Proof of current residence (Driver License copies) - both parents (notarized)
5. Copy of both parents US passports (notarized).
6. Certified check for $185.00
7. Return Fed Express envelope.
Anything else?
