    I need to mail my passport for cancellation today, and was hoping to mail
    my OCI application along with it, so would like to get the below questions
    answered ASAP. I had sent a few emails yesterday and Friday, but haven't gotten responses on all the questions. I would need to mail my Indian passport for cancellation today, so would really appreciate a quick response to my second attempt at getting the below questions answered.
    With regard to the surrendering of Indian passport after attaining U.S.
    citizenship.

    Do I need to fill in the date on the bottom part - "Surrender Certificate"
    of the form? If so, should that be today's date (same as Made and
    subscribed this 7th day of December 2010) or should that be the date I
    took my oath ceremony to become a U.S. citizen (Sept 14, 2010).
    I got my U.S. citizenship on Sept 14'10, so my 90-day period ends on Dec
    13'10. Does the NY consulate need to receive my passport by then, or on
    my mail be post-marked as of Dec 13'10? I wasn't sure if there'll be a
    penalty if they receive it later

    OCI questions:

    my OCI application along with it, so was hoping to get the below questions answered ASAP. With regard to the OCI application form after attaining U.S. citizenship.

    Q.21 on page 3 (part B of the form) asks for details about family in
    India. The form indicates that questions marked with asterisk are
    mandatory. This particular question is not marked with an asterisk, so I
    believe I do not need to fill this out, but would like to confirm that.

    Can you please confirm the same?

    What do you mean by "Self-attested" copies?
    Do I just sign OR
    Do I write "This is a true copy" and then sign?

    I submitted an online application on the website. How do I go about
    cancelling it, as I need to change the Passport number (I typed in one
    extra digit at the end for the number). I want to start a new form, but
    want to make sure the existing form is not used for processing in error.
    Lastly, I won't be able to submit a copy of my cancelled last-held Indian
    passport, as I'm applying for surrendering of the Indian passport form
    (Surrender Certificate) along with the OCI application as a separate set
    today or tomorrow. Would someone in the NY office be providing a copy of the cancelled Indian passport to the OCI part of the consulate to be
    included as part of my OCI application form?

    Do you recommend I surrender my passport for cancelation first, and then
    apply for the OCI, or is it okay to submit both together?

    I would really appreciate a response from you today, as I need to
    surrender my Indian passport by Dec 13'10 so I need to mail it out by
    tomorrow at the latest.
