Incorrect spouse name on OCI

Hello - I submitted OCI application and I accidentally entered my spouse’s last name under her name. It did get flag by embassy and I opted to ignore it for the application to cancel itself, so that I can submit a new one on the return sand refund of it.
To my surprise, I actually got the OCI with her name showing as her last name.

I’m looking for help and guidance on how do I get her name corrected. What does the process look like, what all needs to be sent, the fee etc?
