Minor-Procedure for adding surname in passport

Hello Everyone,

Has anyone here added a surname to their child's passport while living in the USA? My child's current passport only has a given name, and I’d like to add a surname. I’m looking for guidance on the required documents for this process.

For context, my child is still a minor. Any help or advice would be greatly appreciated!

Generally, you will need the child's birth certificate, proof of parental relationship and a completed application form. It may also be helpful to include a letter explaining the name change.
