Hi, Were you able to figure this out, who needs to sign the Surrender Certificate fillable form , parent or minor who is 5+ ?

Also for the Minor Document checklist , it says to "Envelope should be superscribed as “SURRENDER APPLICATION WITHOUT PASSPORT" which is strange, because down the list they want us to include the original Indian Passport.

This is also wrong in the document checklist - "• Must Choose Naturalized Before June, 2010 – Possessing Most Recent Passport."

I wanted to apply last year and they had a different checklist which had all the proper information but now that page which had the pdf doesn't open -

I think the Indian Embassy changed the Renunciation and Surrender of Passport steps and VFS just copy pasted bits and pieces of the previous document checklist and ended up with mistakes.

I too consider these are mistakes and don't intend to put anyting on the envelope.

As well except below the photo (where the minor needs to write their name), and the 2 notarized documents (where both parents need to sign), rest all places only one parent needs to sign is my understanding.

Filling the surrender certificate, there too, guess only one parent need to sign on behalf of the kid.

For my application, when I called them, they asked to type the same content in a word document and send it with proper information. As far as I know this is not used, since my surrender certitifcate was a proper one from the consulate. Anyway we stil need to do it. I opened the PDF content in word and corrected it.

For my kids, I have done the same. Filled the name in first name last name format. But that is yet to be processed, just sent it couple of days back.
