Help on Applying for Lost PIO card without old PIO copy and Number

We have lost my daughter's PIO card along with her passport and we dont have a copy of the lost PIO card. We have got the US passport.
Please let us know the document needed to apply for the duplicate PIO card without the old PIO card copy and number.

Is it possible to apply with one parent passport since my wife also lost her passport which is in process with India Consulate New York.

The Travel date is 2/10/2011. Please help.. Indian embassy dont respond to emails and phone calls
