OCI application Inquiry

hi,
i have a doubt regarding OCI, i want to apply OCI for my daughter (minor, US Citizen by birth) and my wife (US Citizen by naturalization),i am a green card holder ,i am confused which application to use to apply, either individual or family application for them.Please help me.
