    Hi all,

    This is regarding a mistake in my "Given Name" and "Surname" fields in the passport. Instead of splitting my full name into given and surname, my passport was issued with only given name as follows:

    In my passport,
    Given name is : <First Name> <Maiden Surname>
    Surname is : <blank>

    In my current L2 stamp,
    First name : FNU (First name unknow)
    Last Name : <First Name> <Maiden Surname>

    I now would like to have it corrected before applying for Greecard. Also after marriage my name changed to include my husband's name (Though I haven't changed it in any other records except our marriage certificate). I would like to go by my married surname henceforth and I'm planning to change it in all the records soon. I found two options on the Indian consulate site - Splitting Names and Change Maiden name to married. I was wondering what exactly should I be appling for. I would like my name to be changed to:

    In my passport,
    Given name : <First Name>
    Surname : <Surname after Marriage>

    My questions:
    1) If providing an affidavit for only change of maiden to married name be sufficient? Will splitting the name be taken care of in it? OR do I need to provide two different affidavits, one for Splitting name and the other for change from maiden to married?
    2) Can this be done using mail-in service? OR do we need to take an appointment with the consulate and do it in person?
    3) Are we required to publish an advertisement in the newspaper?

    Any help you can provide would be much appreciated.

      Thanks! for the response.
      I did read this page before posting my questions on the forum. The answer to the questions I listed weren't clear to me from the page. There is no mention of advertisements in the list of items to-do. However, in the affidavit there is a note about ads to be posted in Indian and US newspapers.
      In my case, can I just do maiden to married name? Will they split my name too? Or am I required to do need to do both (splitting and maiden to married) Please correct me if I'm wrong in understanding this.
