Fresh Passport - Proof for Change of Name

Need to apply a fresh passport for my mom. She had changed her name. But has lost the affidavit for change of name.

1. Is the proof of name change really required for a new application?
2. If so, how can I use a different proof or get the affidavit re-issued?
