    Hello,
    I am applying to have my passport renewed at the Houston Consulate General, and had a few questions regarding the information to be provided.
    • On the Online Passport Registration form there are two set of columns for the address. My permanent address (i.e. my India address) should be filled out in the “Address to be Printed in Passport” column and my current address (i.e. my current address in the US) goes in the “Other Address ” column. Is that correct?
    • Do I need to photocopy and send all the pages in the passport or just the first 5, last 2 and the visa pages of my passport?
    • Does all the supporting documents(copy of the passport, proof of address and the copy marriage certificate) need to be notarized?
