Hi, i am completing DECLARATION OF RENUNCIATION FORM.I am going to send to SAN FRANCISCO location.So, in colum no. 6 Date of Birth(whose date of birth i should fill).i am thinking my father's date of birth,because in colum 5 they are asking father's name.For applicant(myself)I have already wrote my DATE OF BIRTH in column 1.
Also in column 1 I_______________________(here insert name and address of declarant)am of full age and capacity and was born at(-----------).what should i write about capacity.

Also at last there is some columns in box with heading(SURRENDER CERTIFICATE) i will be fill by CONSULATE.Just want to make sure.

Also in column 1 I_____Only your name and US address will be filled here____(here insert name and address of declarant)am of full age and capacity and was born at(City, State, and Country (For example-Amritsar,Punjab,India)).what should i write about capacity.

everything in the box thats on the bottom must be filled by the consulate. they just stamp it, they dont hand write things. but u must not write or type anything there.
