renew passport - spouse name

I am applying for passport renewal at "Indian embassy at New York" and planning to mail the application.

I was filling the form online:

http://passport.gov.in/nri/OnlineRegistration.jsp?pocode=USAN

I selected "married" but I forgot to specify the spouse name.

1. Can I manually write "spouse name" on the pdf printed form ?
2. Do I have to fill new form online?
3. Is it allowed to fill new for? Will the old form be void.

Hand write in block letters. itx fine

To be on safer is it ok if I fill new form?
As I have made one more mistake. In the "Other address" instead of US address I filled Indian address.

They don't care about the number of mistakes you make. just use a white our and make it look neat and understandable. You can print as many applications as you want but it may delay your appointment date.

why will it delay as I am going to mail the application anyway. I tried filling online again and it prompted for same date

It MAY make a difference if you're doing in person thing at the consulate, but if you're mailing it then it doesn't matter.

I filled another form and corrected the mistake of spouse name and "other address"
