Obtaining a Tourist visa for India from SFO

Hello,

My Husband is a USC (I am an Indian Citizen) and we intend to visit India in december. We intend to purchase our tickets within the next week and will immediately apply for his multiple-entry visa to the travisa office located in San Francisco. Here are my questions:

1) Would we need to submit a copy of the confirmed air ticket?

2) How long does it normally take for the visa to approve/process?

3) Do you know of anybody who has had a negative experience with applying for a visa? If so, what should we beware of?

4) I am a GC holder. What documents will I need to carry when we visit India?

5) Would I need a PAN card in addition to my Indian passport? I have no income in India and do not file taxes there.
