OCI form re-printing

I filled out the online registration and printed out OCI application part a and part b, about a year back. Unfortunately, I misplaced them and was looking to re-print them. However, I am unable to reprint them from the "Re-Print Form"

I do have the reference numbers starting with "USAS...", however, it does not seem to recognize them. Is there some limit on the duration of time, after which the forms filled earlier are discarded?
