  • Affidavit for proof of address for PIO

    I am in the process of applying for a PIO card for my mother who is a citizen of US. One of the required documents is proof of address in US. Unfortunately my mother has none of the documents that is acceptable by the Indian consulate. She does not have driver’s license nor does she have any utilities in her name. US passport or SS card does not have your address on it.
    In such circumstances, will an affidavit by me stating that she is my mother and a dependent and lives with us work? OR if anyone has a better suggestion…

      or you should get her a state ID, it comes in handy.

        Well...I took my chance and went there with a notarized Affidavit stating that my mother lives with me as a dependent. I also attached my birth certificate to establish the relationship and my proof of address. They have accepted the form…will know in 15 days if the consulate accepts it as well.

        If you call Travisa helpline, they’ll say it’s not acceptable....but they do.

        Regarding getting a state ID…that requires a proof of residence as well

          Glad to know the affidavit was accepted. My mom just got a CA ID and they didn't require a proof of address. That's the whole point of having an ID, so you have some address under your name. You just fill out the address on a piece of paper w/ some sort of ID (passport/green card/ssc work) and they issue a state ID. They don't require proof of address to issue an ID. But on the other hand, I have no idea what state you're in.
