passport renewal

Hi,

I am in the process of renewing my Indian passport which is going to expire . I just moved to Nebraska from Ohio on a short term project. I am currently holding Ohio's driver's license and planning to change it to Nebraska license.since Nebraska comes under San Fransisco Indian Consulate jurisdiction, it saying to submit one of the driver's license as proof of residence which I am assuming one should be good enough.But when i checked immihelp it is mentioning about two proof of residence which is confusing.I am just wondering how many should I submit for my application to be accepted in the San Francisco.Could anyone please advise asap.

original passport.
