Do I need a name change or split ??? Help !

Hi everyone, my current Indian passport has the name from my birth certificate, in the following format:

Surname: A
Given Names: B C

The format I want, to appear in the passport, is the following (because, in the US, this is the format I have used for 10 years):

Surname: B
Given Names: C

Note that the modification I want involves 2 things:
1 - Removal of a name (A)
2 - Moving a name (B) from the "Given Names" field to the "Surname" field

Which type of application do I need to submit (SFO consulate) ? Change of name OR name split ?
