Name change petition

Hello,
I received a letter from CGI houston stating that the name in my indian passport and US passport do not match. Basically I have a middle name in my Indian passport and the US passport does not have it.
So they want an 'Name change petition duly notarized' as the names in the passports do not match.

What is this name change petition? Is there a specific form? Do I need to go through the whole legal name change process?
I sent 2 emails to CGI houston but got no reply.

Thanks for the information. I have a couple of questions regarding this.
- Does the name change need to be done in India also? (ad in Indian news paper etc.)
- How long does this process take
- Will CGI houston hold on to my application until I get all the necessary stuff taken care of? DO I need to inform them that I am going throughthe process? If so, how?
- Does Immihelp have any legal services that will do all the necessary stuff for a fee?
