Docs to send with OCI app, confusion? Help!

- Do I need to send the Indian passport at the time of sending the OCI application or after OCI granted along with US passport?

- For name change do I have to write a notarized letter or the copy of court order is sufficient?

- if any of you applied OCI for Indian born minor, can you paste text of your notarized letter (explaining situation in which citizenship acquired)

3. Situation citizenship acquired does not have to be any elaborate reasons. You can write whatever the situation is such as parents got the citizenship, or child was born in the U.S. etc.
