Need help for Name Change document for OCI.

I can attest that i used a similar sample format letter that Need_oci posted. I have a similar issue where my indian password had my middle name and my US password did not. I was trying to apply for a 5 year business visa and this issue came up. I used the similar content as Need_oci and used my naturalization certificate # showing my middle name. you can justify by using your birthday on all documents to relate that you are the same person. Did any one had similar issue as mine and received any update ?

I've an issue where "bahen" was added behind my wife's name and We changed spelling and removed bahen in my wife's name during citizenship and I have the "Petition for Name Change" from USCIS for the same.

I didn't have any issues while Renouncing the passport, but just confused why they make it so hard, but please let me know if anyone got thru similar case.

Just an update.

I received the approval for OCI based on an earlier post' details, I send notarized affidavit along with proofs of School Leaving Certificate and Birth Certificate.

NEW OCI Application:
I got my name changed (to Husband's Middle & Last name) during Naturalization, so my cancelled Indian passport (maiden middle & last name) & US Passport now have different name. Do I need to submit Name change Affidavit or court order provided during Naturalization will work ?
