Passport Reissue

I'm currently in US and planning to send my passport to Indian Embassy in Chicago to change the last name. I believe they will issue a new passport and attach the old passport to it as the visa in on old passport. My question here is if we need to take out the I94 from old passport before submitting or have to keep it on the passport?

You take out the I-94 form. They will not attach new passport to old passport or vice versa but they will give you old and new passport.
