PIO application for child born in India to US citizen parent

  • PIO application for child born in India to US citizen parent

    My son was born in India and acquired US citizenship by birth as his father is a US citizen, me( mother) is Indian citizen with US greencard.I would like to apply for his PIO and has few doubts about filling the application form.
    1.Is permanent address ( Q 14) mandatory to be filled with?
    2.For Q 17, nationality at time of birth, i believe its USA as my son was born to US citizen parent. Pls correct me if it should be different.
    3.For Q 25, since the applicant is an infant ( minor) can i attach one of parent's drivers license or utility bill under father's name as proof of current address?

    Should i mail my child( applicant's) original passport along with order form and PIO application form, as i read somewhere that we dont need to send original passport unless we receive confirmation email

      Thanks for the response. But Question 14 specifies address in India. I understand that my son's(applicant) permanent address is US address, but should we give his grandparent's address in India as its mentioned (verifiable) ?....

        In the PIO requirements it is asked for copy of Naturalization certificate
        "If a minor gained naturalization through his/her parents and possesses no separate naturalization certificate, then copies of the parents' naturalization certificates and a notarized written statement signed by both parents is required stating the circumstances that their child received his/her naturalization status and why the child does not possess his/her own certificate"
        My son has "Consular Report of Birth Abroad" certificate that was issued at Chennai US consulate when we applied for his US passport.So i assume since he has no specific Naturalization certificate, how do we provide written statement notarized( can it be printed with above stated matter and signed by both parents ( assuming signed by us is considered notarized) OR should it be attested or notarized officially from someone who does notary? with a stamp or so?.......


        What are the pages in passport of US citizenship parent that is required to sent as photocopies. I understand that the indian passport pages with details and renunciation stamp and US passport page with picture and details are needed, but its mentioned as pages 22-28 if US/ Indian passsport in PIO requirements which is not very clear, are any of these pages too to be sent as copies?

        The passport copies of parents is also asked under "Proof of parents Indian origin" is it only the front 2 and last pages with details in Indian passport?

        For Q24 in PIO application form

        Whether the applicant or his/her parents or grandparents
        or great grandparents was born and was permanently
        resident in India as defined in the Government of India
        Act, 1935 * at any time {please see instruction (1) }. If
        so, following details may be indicated:
        (b) Date of birth (DD/MM/YYYY)
        (c) Place of birth
        25 Proof of residence etc

        Do we need to provide proof of residence for both parents( if so what should be provided as proof) as there is space for only one parent info. These are quite confusing..
