Foreign Life Insurance - Excise Duty

Hi, I wanted to know what is the exact procedure and requirements to File Form 720 for Foreign life insurance. As a US resident if we have any policy in India we need to pay 1% Excise Duty with Form 720 or file Form 8833 to claim exemption on the same. Does anyone know what are the intricacies involved, to file Form 720 would I need to have a Employer Identification Number (EIN) ?
