Spouse last name change

Hi,

My wife has her maiden name in her passport and H4visa as well whereas her medical records here have my last name.
Would this be an issue while applying for child passport or PIO ?
If yes how to change wife's maiden name to my last name in passport and H4 visa?

Not an issue. It is in your best interest to maintain the same name in all places. Married name change details on passport are available on immihelp home page. Check it out.
