Procedure to get Passoport for infants born India

Hi Group,

I'm blessed with baby boy. He is in India and I'm here in the US. I want my kid's passport to be ready for visa process. Brokers in India are asking to send the SWORN certificate from the US embassy. I have no idea how to get the certificate in the US. Some says I need to fill the form and send documents to the embassy. Can you please send me the correct link to fill the form and other information related to certificate.

You have not specified the citizenships of both parents and where they permanently live.

What is the visa (or any other legal) status of both parents in the U.S.?

I came to the US in April 2011. I have L1 visa. I live in Florida at present. Both my wife and I are having Indian citizenship. My wife is now in Mumbai. I want to bring my faimiy here. I need to get the passport for my new born son. Please let me know what is the procedure to get it.
As I came to know it requires some affidavit from the US consulate to provide the passport in India.

I got some infor from this site. Please advise me.
