Virginia health insurance application

I applied for Blue Cross Blue Shield health insurance and was approved for myself but not for my 18 month old son because of ‘previous conditions.’ I was referred to Standard through that process. My question is, since I am already approved for another plan, how do I apply get coverage for him? Is this application (the PDF) a separate or an addendum to mine? Also where do I mail this application to?
