need insurance residing in India and US

One of our customer wants medical insurance for the family. It should cover in USA, Canada and India for the whole family ( 4 members). Do you have any such plan which can cover in both countries. Customer family has US citizeship as they were there for 15 years, by birth they are Indian citizens. Please suggest you have any comprehensive plan to suit the requirement.

They will be staying approximately 6 months in US & 6 months in India, not at a stretch, but on & Off. Annually renewable insurance is preferred, because any point of time one person of family will be in US.
