I am not aware of any insurance plan that would completely meet all of your requirements.

The closest match is Study USA-HealthCare and you can find it at https://www.insubuy.com/study-usa-insurance/
Many times, university will accept the one that closely meets the requirements.

      Hi...

Could you make sure that the insurance plan includes: Max. deductible of $100 per conditon per plan year. Max. out-of-pocket expenses $2,500 or less. because after I purchase, I need the insurance company or agent to have affidavit to my school, San Francisco State University.

Please let me know as soon as possible.

Please refer to my previous email.

Study USA has $50 deductible per incident with max deductible of $250/year. However, deductible is waived if you go to the student health center first or if you go to the provider in the network if there is student health center.
Therefore, your university may still accept this.

You have to pay 20% for first $25,000. That is $5,000, which is more than $2,500.

That is why I asked you to check with your university whether they will accept this plan or not.
        Thanks & Regards
