I am Pratt student and I purchase the health insurance from your company
as I want to waive my Pratt 's health insurance ,I need to go online and waive .
In the process of internet I need to fill up the form ,but I don't know how to do such as

I only found the United healthcare on the internet ,so I don't know what is exactly the name of your company.

as far as I known ,I took Plan A ,but I still can't find any plans are related to my health insurance .
