We are interested in Plan A for visit usa ($50,000 Illness and Injury Medical Expense/Incident) with Deductible Per Policy Period of $250. Please verify that the policy for my J-2 dependents meets with the following requirements:
1. Medical Benefits of at least $50,000 per accident or illness;
2. Repatriation of remains in the amount of $7,500;
3. Expenses associated with the medical evacuation of the Exchange Visitor to his/her country in the amount of $10,000;
4. A deductible not to exceed $500 per accident or illness (also may include a waiting period for pre-existing conditions and 25% co-payment);
5. Policy shall not unreasonably exclude coverage for perils inherent to the activities of the Exchange Program in which the Exchange Visitor participates;
6. Must be acceptable in all medical facilities.
Other questions:
Premiums are Monthly or do I have to pay all period in 1 pay down with credit card (because it means that I have to pay $2,568 in one payment)?
