Monthly payment for J1 visa insurance

I am 39 years old. I intend to visit US on J-1 Visa along with my wife who is 33 years old and a child who is 8 years old (both of them on J-2 status). Kindly advise me the plans available and premium for one year. Also let me know if the premium can be paid monthly. If yes, will there be any limitations coverage / reimbursement. Thanks in advance
