1. before i give out the news paper adv, do i need to get it approved/attested by Indian Embasy in US? in india?

2. Here is the Adv format. is this right?
I _________________ S/o ______________ r/o ( Please write here your address) holder of Indian passport no. ___________________dated ________ issued at _____________ intend to declare that I have changed my name as ________________________ __________ for all purposes.

3. will this process make any problem in my green card process?
