Indian Passport Renewal(NYC) question! Please Help

I am planning to renew my Indian passport through NYC consulate by mail.I need clarification regarding two questions in the online application form.

Address to be printed in the passport. What should i give here USA or Indian address?
Other Address. What should i give here?

How does the consulate know which address to mail the passport back? I didnt see any specific details regarding this in the application.Should i inlcude a cover letter to be safe?

Called up NY CGI on similar question.

Addr under 3.(a) on the renewal form should be Indian address as mentioned in old passport. Addr under 3.(b) should be a current US address. The address proof is needed for 3.(b).

In case you have new Indian address, you need to submit second address proof for changing Indian address too.

Since passports are never to be send across a border in a mail, CGI should send it on US addr only.
