Passport renewal at DC embassy

Hi,

My Indian passport expires in Jan 2011. I am applying for renewal by mail at DC Embassy. I have couple of questions:

1. In the online application for PP, there is a field Service Desired. What should be selected here, Fresh Passport OR Re-issue of Passport?

2. In the Address To be printed and Other Address field, should the US address or Indian address be entered?

3. What are the documents to be mailed with the application?

This is very urgent..Appreciate quick replies.
