Birth Certificate

Dear Sir/ Madam,

May you coould help me on this, I got born in Visakhapatnam in 1979 and do have my birth certificate (Form 9) but do not have my name on that, wondering if you could please help me in getting or let me know the procedures geting a new birth certificate with my name on it.
