need help with indian passport renewal in ny- asap

hello,
i want to renew my indian passport in ny & have some questions..pls don't direct me to the related pages..i have been thru the procedure here & also the official ny counsulate site..

i have a indian passport which was issued by tatkal for 5 yrs in india in 2004.
i mentioned my place of birth as xxx (local area) instead of city..my dh wanted me to change it so i did.they din't change the print on pp but they added a statement saying that the person's place of birth is changed to read as xxx & stamped & signed by assistant to pp officer...my pp expired in 2009i'm gc holder..

i'm pretty sure i need a renewal form but want to make sure as its been 2 yrs since expiry( i din't realise it till nov last year)

1) do i need a renewal form or a new passport issue form ? does it have to be notarized ? the normal new pp appli form need to be if xpired more than a yr

2) do i need a form for change in place of birth ? does this have to be notarized too ?

3) what do i mention as my place of birth in the form ?

4) one of the fields in nationality status forms says " date of leaving india " & "period stayed abroad "..does this mean the first time or last time of leaving india & total stay or continous stay in usa ?

5) how many pics do i need in total ?

6) i filled the online form, can i fill the rest of the pages in pen or do they have to be printed out ?

7) local car driving license - indian licence or usa ?

8) what do i mention the reason for delay as ( more than a yr xpiry )

can someone pls provide me with correct info as i have to drive 3 hrs to ny counsulate & don't want to go again !

New passport issue form would be required. It has to be notarized if you apply by mail. If you drop in at the consulate physically then no need to notarize.

No need for the form. as the change is already mentioned in your previous passport as a note. (if that's not the case, then it might be required. email the consulate.)

3) what do i mention as my place of birth in the form ?
Your correct city of birth that's written on your birth certificate.

Date when you first left india and how long have you been living abroad.

take 2

yes you can change them with a pen with block letters

USA license

How would I know that. It's your personal understanding. Ask yourself.
