Indian passport for child away from one or both parents

Hi,

I need to apply passport for my daughter ( < 2 years) in India, my family is in India and I am in US on B1 visa for short term trip. Looks like we need to send a letter to Indian embassy in US and get it attested and then send to India to submit along with passport application?

I have got some information from this web site but not sure what to submit for proof of status and proof of residence as i am here on B1 visa. Can you please tell me what documents i have to submit to get NOC from consulate.

I am calling several times to Indian consulate but as usual no response.

Help is greatly appreciated.
